Enter a “Name” (Required).

Select whether this type of equipment will be “Active” or “Inactive”.

Enter “Frequency” of the reporting on this type of equipment (Optional).

Select the “Duration” of the frequency (Optional).

Click here to create your “Equipment Type”.

Step #10

When the form is complete, click here to create your “Equipment Type”.
